CHART # 1 of 4: 
Improvement in Ambulation/Locomotion

                                                                               How to read this chart: A higher score is better



Measure Definition:This chart shows the percent of clients who improve at walking or moving around in a wheelchair safely.

Why is this important? Improvement at walking or moving around lessens the risk for falls and/or injury and allows for greater independence.

What are we doing to improve this outcome? Holy Redeemer HomeCare assesses each patient’s ability to move around safely.
Patients are referred to Physical Therapy for instruction on use of assistive device(s) and training in strengthening and endurance exercise programs.



CHART # 2 of 4: 
Improvement in Management of Oral Medications

                                                          How to read this chart: A higher score is better



Measure Definition: This chart shows the percent of clients who improve at taking their medicines correctly. The measure includes only medicines the patient takes by mouth.

Why is this important? For medicines to work properly, they need to be taken correctly. Taking too much or too little medicine can slow recovery or have other unexpected consequences.

What are we doing to improve this outcome? Holy Redeemer HomeCare instructs all patients or their caregivers about the use, frequency, dose, time, therapeutic effect, and side effects of their medications. All patients are provided with a written medication list.
The goal of our medication teaching is that the patient or caregiver is able to self-manage their medication regimen safely.


CHART # 3 of 4: 
Acute Care Hospitalization

                                                                      How to read this chart: A lower score is better



Measure Definition: This chart shows the percent of fee for service Medicare patients who are admitted to the hospital within the first 60 days of a home care episode.

Why is this important? The need for readmission indicates deterioration in a client's condition - either from the original condition or a new condition. While not all hospitalizations can be prevented, consistent care and observation can help reduce hospitalizations. 

What are we doing to improve this outcome? Holy Redeemer HomeCare instructs all patients about signs and symptoms of potential problems and what to do if problems occur. An emergency care plan is provided and reviewed with the patient or caregiver. Patients may contact Holy Redeemer HomeCare 24 hours a day.

CHART # 4 of 4:
Emergent Care for Injury Caused by Fall or Accident at Home


                                                                  How to read this chart: A lower score is better



Measure Definition: This chart measures the percent of clients who fall and must go to the emergency room. The client may have been treated in the emergency room and sent home or may have been admitted to the hospital for their injuries.  

Why is this important? Falls, particularly in older patients, can lead to serious injury or even death.

What are we doing to improve this outcome? Holy Redeemer HomeCare screens all patients for their risk to fall. The patient may be referred to Physical Therapy or Occupational Therapy. Patients are provided with an instruction booklet about risk for falls and how falls can be prevented. A toolkit containing a night light, a flash light, non skid socks and a whistle are provided to the patient. Additional adaptive equipment may be recommended.
