CHART # 1 of 6 (Holy Redeemer St. Joseph Manor & Holy Redeemer Lafayette):
Urinary Tract Infection (UTI) Rate

                                                                                         How to read this chart: A lower score is better



Measure Definition:This chart shows the percentage of residents that had symptoms and a positive laboratory test of a urinary tract infection.

Why is this important? If symptoms go unidentified or untreated, these infections may lead to more serious events such as falls, confusion or more severe infections.

What are we doing to improve this indicator? All infections are tracked and evaluated by infection control nurses. Additionally, we educate staff, residents, and families on the importance of proper hygiene and hand washing techniques.


CHART # 2 of 6 (Holy Redeemer St. Joseph Manor & Holy Redeemer Lafayette):
Pressure Ulcer

                                                                         How to read this chart: A lower score is better



Measure Definition: This chart shows the percentage of residents who had developed a wound from pressure while in the nursing care facility.


Why is this important?
 Prevention and early detection of wounds can decrease the risk of further complications.

What are we doing to improve this indicator? All residents are assessed on admission to identify their risk. Those residents at risk for pressure sores then receive preventive measures and care and team members meet regularly to monitor treatment and progress.


CHART # 3 of 6 (Holy Redeemer St. Joseph Manor & Holy Redeemer Lafayette): 
Falls Resulting in Moderate or Major Injury

  Fall Rate does not have a national or state benchmark. Holy Redeemer strives to have less than three falls per 1,000 resident days.



Measure Definition: This chart shows the rate of resident falls per 1,000 resident days that resulted in moderate to major injury .

Why is this important? Falls, particularly in older residents, can lead to serious injury or even death.

What are we doing to improve this indicator? Our nurses assess all residents on admission to identify those at risk of falling. Those who are at risk for falls may receive assistive devices, evaluation by a therapist, walking programs, as well as other measures to help prevent falls. If a fall does occur, staff meetings are held immediately to develop a plan for care and determine what can be done specifically to prevent a future fall in that resident. 
 

CHART # 4 of 6: 
Drueding Center Residents Who Left Transitional Housing and Went On to Permanent Housing
                                                                                           
                                                                              How to read this chart: A higher score is better


Measure Definition: Upon departure, the family is moving to a location in which they can remain for the foreseeable future.

Why is this important? This is a widely accepted measure of success for transitional housing programs and one closely monitored by the Department of Housing and Urban Development. Moving to a permanent location provides stability for a family and helps to promote overall well-being.

What are we doing to improve this indicator? Drueding Center is advocating on behalf of our families for increased access to subsidized housing programs. We are also working with families who are at risk for being dismissed from the program (families dismissed from the program are at an increased risk of moving to a temporary or unknown destination).


CHART # 5 of 6: 
Drueding Center Residents Employed at Exit

                                                                                 How to read this chart: A higher score is better

Measure Definition: Upon departure, women are engaged in a paid work experience. 

Why is this important? Employment income is essential to the self-sufficiency of our families. Not only does it provide for income in the present, but also allows women to gain valuable work experience that will help them to progress in the future.

What are we doing to improve this indicator? Workshops on employment-related topics (such as interviewing skills) are provided on a regular basis.  Family advocates assist women with employment-related goals as a part of the Family Service Plan and pass along leads on employment opportunities as they become available. 


CHART # 6 of 6:
Drueding Center Active Graduates Maintaining Independent Housing for Three Years

                                                                             How to read this chart: A higher score is better
                           

Measure Definition: Three years after a family has exited they are in a stable housing situation.

Why is this important? Multiple moves have negative consequences for a family and are a strong predictor of recidivism in the homeless services system.

What are we doing to improve this indicator? Family Advocates assist families in creating a budget and locating housing that remains within budget. Families exiting from Drueding Center can enroll in an optional After Care program in which they receive on-going case management services for up to seven years.
