Holy Redeemer LifeAssess ProgramHoly Redeemer's LifeAssess program is a comprehensive assessment, education, support and rehabilitation program that identifies and treats older adults who are at risk for four of the most common geriatric syndromes:

  • Depression
  • Dementia
  • Falls risk
  • Incontinence

These common syndromes may threaten the ability to live safely at home—leading to premature placement in a nursing or long-term care facility and frequent hospitalization. Our goal is to allow those who are aging at home and at-risk for these conditions to remain as independent as possible and to live safely and comfortably in their homes.

Could you or a loved one benefit from LifeAssess? Ask yourself or your loved one these questions:

  • Have you had decreased interest or no interest in doing the things you used to enjoy?
  • Do you have trouble answering questions like: "What day of the week is it?" Or "When were you born?"
  • Do you suffer from urinary incontinence or wear protective pads?
  • Do you have trouble walking even short distances?

A positive response to any of these questions indicates that you or a loved one may benefit from a referral to LifeAssess.

Individuals who are already patients of Holy Redeemer’s HomeCare program, automatically receive a comprehensive screening and evaluation, complete with recommendations and an action plan. However, this innovative screening and assessment program is open to anyone with a physician’s order, and, in some cases, verification of homebound status and insurance approval
